At first glance, William's middle initial does look like "G," but if you check other entries to see how the enumerator wrote the letter, "D," it is possible to read it as D, which it surely should be.

Mary (KAUFMAN) DOSHBAUGH is the mother of Mary (DOSHBAUGH) CROUCH.  At the time, "insane" was a blanket term that included old age dementia, alcoholism, retardation, etc., anything that made someone mentally abnormal.  Jimmy DEFORD appears to be a brother of William's late wife, Harriet (DEFORD) COUCH.

Mary Ann "SURFACE" is almost certainly Sallie's sister, Mary (DOSBAUGH) CROUCH ANDREWS, erroneously enumerated here with surname SURFACE.  Mary ANDREWS died in West River Township in 1912, and Sallie SURFACE was the informant on her death certificate (see below).  Sallie (DOSBAUGH) SURFACE is the mother of "Cousin Mary."

12.  Indiana State Board of Health.  Indiana Death Certificates, 1899-2011.  Indiana Archives and Records Administration, Indianapolis, IN (online at Ancestry.com):
Mary's parents are indexed "Dorbaugh," but I would read it as Dosbaugh.  The Informant is almost certainly Mary's sister, Sarah (DOSBAUGH) SURFACE.
