They are at the top of the page, and clearly p. 11A should have been the page after 10C, that is, 10D, not 11A.  The visitation numbers are equally screwed up.  The next sets of visitation numbers on this page are 7/1, then 8/2, then 9/1, then 240/4, and so on...
