Warning:  our subject is being confused with the George W. SURFACE (29 Dec 1836 - 22 Jun 1904) who was son of David SURFACE & Catharine PRUNER and who married Elizabeth Jane BOWEN — and his middle initial is "W," not "H."  In the 1870, 1880, and 1900 Censuses, G.W. SURFACE is in Boone Co., IN, while our subject, G.E. SURFACE, is in Johnson Co., IN, in 1870, 1880, and 1900 (see below).  Clearly, these are two different men.

There's a 15-year gap here. 
Is George Edward really their biological son?

George's marriage certificate gives his father as George E. SURFACE and his mother as Sarah DAUSBAUGH, though that doesn't rule out an adoption.
Children often weren't told they were adopted, so there's little reason the informant on their death certificate would know.

Likewise Ethel; I don't know what to make of her age (see below), and the census shows her mother born in Indiana, not Ohio.

I don't know what the nature of this record is or why it's in with the "marriage" records, but it does give us our subject's middle initial.

I cannot fathom what age Ethel is intended to be.  Also, her mother is shown as born in Indiana, not Ohio.  Is she adopted?

"Mary Ann" is almost certainly Sallie (DOSBAUGH) SURFACE's eldest sister, Mary (DOSBAUGH) CROUCH ANDREWS, who was her only sibling born before their parents emigrated from Germany.  Mary ANDREWS died in West River Township in 1912, and Sallie was the informant on her death certificate.  "Cousin Mary" is Sallie's daughter.
