The image is obviously from a book, not only based on the typography of the caption, but because the half-toning is evident in the photocopy.  I feel compelled to apologize for the dreadful quality of the image, but it is, in fact, a good scan.  It's the photocopy that's dreadful — obviously photocopied many times over.  I'm actually surprised it turned out as well as it did.  I will try to find the source and get a better image to post.  If you know the source, please let me know.
